Addiction is a chronic disease that requires ongoing support and treatment even after undergoing initial treatment programs. Approximately 40-60% of people in recovery relapse, and so it is important to have a planned course of action for managing the risks of relapse. Aftercare planning plays a critical role in effectively supporting continued recovery, and Penfield Addiction Ministries helps to develop individualized after-care plan.

What is Aftercare Planning?

Aftercare planning is the process of developing an individualized plan for continuing care after completing a treatment program. It is a proactive approach that helps to provide the necessary support and resources that enable individuals to successfully maintain their recovery. Typically, aftercare programming involves ongoing support, finding resources unique to the client, sober living housing, and education.

Aftercare Planning in the Penfield Addiction Ministries

At HEART, we recognize that aftercare planning is an essential part of addiction treatment. Our comprehensive aftercare program is designed to provide customized support to meet the unique needs of each client, and aims to prevent relapse, promote recovery, and support long-term success.

  • Individual Counseling: We recommend individual counseling once you leave treatment. We will give you several referrals so that you can make the right choice for you.

  • Sober Living Housing: We offer sober living housing for women transitioning from primary treatment programs. Our sober living housing provides a structured environment for achieving independence and for gaining the needed guidance and support to prevent relapse.

  • 12-Step Support Groups: While in treatment you’ll experience what it’s like to attend a 12-step Support Group such as Alcoholics Anonymous or Narcotics Anonymous. This will help you overcome being anxious about going to an outside meeting. We’ll give you a list of meetings in your area so that you’ll be prepared when you go home.

  • Relapse Prevention: Our relapse prevention classes during treatment focuses on identifying warning signs and developing a plan of action in case of potential setbacks. You will develop you relapse prevention plan before leaving.

  • Home Environment: We’ll help you assess the problem areas in your home environment, make plan of action and help you communicate with your family.
